For Forklift University to process your operator license, you MUST submitt a head shot photo, (SELFIE). You can use this form to select a picture from your computer, or if you are on your phone, you can take a selfie and upload directly. Please fill in the information in the form before clicking on submit. Files must be less than 1 Meg in size and must be a JPG or a PNG. No other formats are accepted.
